Two forms with the same ID

Hello everyone,

i've getting the same form displayed twice.
In database, only one 58 no problem. Do you know what can cause this ?

Strange.
Did you try to clear all Joomla cache and browser cache?
Is this form set to "Recording in database" ( in Form processing, default is YES)?
Do you have one or more lists with this form_id (check in #_fabrik_lists)?
