Both of these are unmoderated. It will be up to both parties to work out the details and come to an agreement.

When I first looked at this plugin in August of 2012, it was working marginally but still had issues. It had the potential to provide some awesome functionality so I integrated it into my site as you noted. Unfortunately at this point in time, it isn't functional at all for me but it is still a live issue: http://fabrikar.com/forums/showthread.php?t=31021
I think it is something Fabrik wants to do at some point, just that F! 3.1 takes precedence. There are a couple of other users and silver subscribers that I really hoped could jump-start things but I'm not sure even where that sits right now. I can't justify a silver subscription of my own so I may end up disabling it until some development time can be spared. Disappointing yes, but understandable with the limited resources.
