I have experienced what seems to be a bug with the 'form' stuff

I installed
Joomla 3.3.1
Fabrik 3.1
PHP 5.4.x (not sure of last digit)
Tried locally and on hosting provider

ISSUE:
I created a List, which automatically creates a Form and Group
I then created an element (type=field), associated it with the group.
All good so far.
When trying to view the form and add data in the back end the ADD and CSV buttons were not visible.
On the front end both Form Menu Item and Form Module would not display - a warning message said I didn't have permissions
At OSTraining, Nick sorted it for me by changing the form template to Bootstrap, saving, then changing back to default. All now seems good.
ADDITIONAL INFO:
When I installed the sample data the form in the backend showed the ADD and CSV buttons, but threw up a 500 error on the front end.
The 500 error disappeared when I unpublished the module.
