It is just a Fabrik form using the parameters provided for the element. Had not made any changes to the form. It has no java or PHP script. Yes cleared the browser and used different browsers. Yes, a join element. Thank you for checking into this!
I restored to the previous GitHub master and it works as it should ... it was the new Github update from Saturday morning that prevented the popup modal.
