Data Grouping is Inconsistent

I have a table of children and each child is linked to one or more guardians in another table. I'm getting very bizarre inconsistencies when viewing the list. See this:

#1 and #4 is how the data should show up
#2 is combining one record, but the other is separate
#3 is not combining any of the records

Seems so strange to me...I've looked at the db table and can't see any differences between the records that display correctly and the records that don't. I've been banging my head on this for the last week and it's really frustrating. Does anyone have any idea what is going on here???

Your list seems to be sorted by your 3rd column.
Records are only merged (combined) if they are side by side, so if you want to display one child with his guardians merged you must sort (order) your list by child (id).
