cannot populate the cdd element with data from database table

Please check the cdd element, after today update i cannot load the data from database table. Tested with IE9, Chrome, FF, website and browser cache has been cleared, still i cannot select the list from dropdown..

Few more info using Chrome debug:

Uncaught ReferenceError: Fabrik is not defined fabriktables.js line 122

Line 122 is:

Fabrik.requestQueue.add(myAjax);
